By accessing or using ChiroMatch ("the Platform"), you agree to be bound by these Terms of Service. If you do not agree to these terms, do not use the Platform. ChiroMatch is operated by ChiroMatch ("we," "us," "our").
ChiroMatch is a clinical screening and provider referral platform. It helps patients assess their suitability for various chiropractic modalities and connects them with providers who offer those modalities.
ChiroMatch is not a healthcare provider. The screening tool does not diagnose conditions, prescribe treatments, or replace a clinical examination by a licensed professional. Results are informational and intended to guide conversations between patients and their healthcare providers. If you are experiencing a medical emergency, call 911.
The screening tool is free to use and does not require an account. By using the screening tool, you acknowledge that the results are generated algorithmically based on your self-reported inputs and do not constitute medical advice. You are responsible for the accuracy of the information you provide.
When you request an appointment with a provider through ChiroMatch, you consent to sharing your screening results and contact information with that provider so they can follow up with you.
Providers must create an account to be listed on ChiroMatch. By creating a provider account, you represent that you are a licensed chiropractic professional (or authorized representative of a chiropractic practice) in good standing in your state of licensure. You are responsible for maintaining the accuracy of your profile, including your modalities, address, and contact information.
ChiroMatch reserves the right to suspend or remove any provider listing that is found to contain inaccurate information, that violates these terms, or at our sole discretion.
ChiroMatch offers free and paid provider listing tiers. Paid plans are billed monthly via Stripe. By subscribing to a paid plan, you authorize us to charge your payment method on a recurring monthly basis until you cancel. You may cancel at any time from your dashboard; cancellation takes effect at the end of the current billing period. Refunds are not provided for partial months.
ChiroMatch facilitates introductions between patients and providers. We do not guarantee that any referral will result in a booked appointment, and we are not a party to any provider-patient relationship that may result from using the Platform. Providers are solely responsible for their clinical decisions and patient care.
All content, design, code, and branding on the ChiroMatch platform are the property of ChiroMatch and are protected by applicable intellectual property laws. You may not copy, modify, or distribute any part of the Platform without written permission.
ChiroMatch is provided "as is" without warranties of any kind, express or implied. To the maximum extent permitted by law, ChiroMatch shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the Platform, including but not limited to clinical outcomes, missed referrals, or billing errors.
You agree to indemnify and hold harmless ChiroMatch, its officers, employees, and affiliates from any claims, damages, or expenses arising from your use of the Platform, your violation of these terms, or your violation of any third-party rights.
These terms are governed by and construed in accordance with the laws of the State of California, without regard to conflict of law principles.
We may update these terms from time to time. Continued use of the Platform after changes are posted constitutes acceptance of the revised terms. Material changes will be communicated via email to registered providers.
